QualificationsLicense/Certifications:
- Valid IL Driver's License upon hire.
- MVR ran annually.
- Bachelor's degree in social work, psychology; related field
- Licensed Practical Nurse (LPN) A practical nurse who holds a valid license in the state of practice
- Certified Rehabilitation Counselor (CRC) Certificate of psychiatric rehabilitation from a DHS-approved program, plus a high school diploma or GED, plus two years' documented experience in providing mental health.
- Certified Recovery Support Specialist (CRSS) Certified Recovery Support Specialist.
ResponsibilitiesSpecialized Knowledge and Skills Requirements
- Ability to delegate wisely, exercise good judgment, maintain good interpersonal relationships, adapt to change, evaluate critically, and think creatively.
- Demonstrates knowledge and skills necessary to provide appropriate care to clients with severe and persistent mental illnesses.
- Effective written and verbal communication skills for interacting with physicians, co-workers, other clinic/hospital personnel, and the public.
- Good organizational skills.
- Provides community-based/in-home behavioral health services for individuals experiencing serious mental illness that interferes with their ability to manage mental health symptoms and live independently.
- Provides transportation to clients as clinically appropriate.
- Completes assessment and treatment plans with clients. Requests consultation, where indicated, in a diligent and timely manner.
- Provides on-call interventions for ACT clients, covering after business hours, overnights, and weekends as assigned.
- Provides crisis assessment and intervention, completes safety plans with clients, and follows up with clients following crisis situations. Coordinates with Emergency Response Service and other crisis service teams when clinically appropriate.
- Provides individual and group ACT services, including monitoring daily living activities, assisting with benefits and entitlements, employment and education support, symptom management, medication observation as requested by physicians, advocacy for clients' rights and preferences, resource access assistance, family support, and coordination of services and care.
- Maintains records in accordance with UnityPlace policies, as well as state and federal oversight agencies and accreditation agencies including Joint Commission, Medicaid, Medicaid Rehabilitation Option, Illinois Department of Public Health, and Commercial Insurances.
- Acts as a liaison and resource between the community and client needs; coordinates treatment goals with community agencies, primary care physicians, court system, and managed-care entities.
- Participates in in-service and competency training.
- Participates in interdisciplinary team meetings.
- Prepares special reports as needed on activities and work group activities.
- Meets clinical productivity standards.
- Meets compliance standards for all documentation and clinical charts.
Compensation and BenefitsThe compensation range for this position is $18.39per hour - $29.79per hour. This represents a good faith minimum and maximum range for the role at the time of posting by Carle Health. The actual compensation offered a candidate will be dependent on a variety of factors including, but not limited to, the candidate's experience, qualifications, location, training, licenses, shifts worked and compensation model.
