Primary Care Coordination Health TechReports to: Primary Care Program Manager Status: Non-ExemptLocation: Crook County Medical Services District Job Type: Full-TimePOSITION SUMMARYThe Care Coordination Health Tech is a full-time position responsible for assisting with coordination of health services to patients to keep them healthy through wellness and prevention. This position will serve to support the CCMSD Care Coordination programs as the programs continue to grow. At CCMSD we are uniting industry-leading solutions to build an integrated care model that addresses an individual's physical, mental, and social needs. Our goal is to help patients navigate and connect care to create a seamless health journey for patients across care settings. The Care Coordination CNA/MA will function in a hybrid position that includes working remote, on-site, and in patients' homes. This position will support the development of patient care coordination, appointment scheduling, referral processing/follow-up, medication review, maintaining medical records, patient/family/facility communication, helps patients by supporting personal hygiene and daily living needs, transportation, and vital sign monitoring. Provides high-quality and cost-effective personalized care to residents/patients and performs duties under the direction of the care coordination charge nurse.Responsibilities
  • Detailed review of EMR record to inform initial outreach.
  • Monitor patient's ED visits and acute stays, perform post-discharge follow up calls and continuously assess risk of readmissions post-discharge.
  • Utilize motivational interviewing to promote patient engagement and empower patients to develop self-management skills.
  • Communicate proactively with provider to address patient change in status or obtain any necessary referrals/orders.
  • Provide medical documentation in our care management software system.
  • Develop and maintain effective professional working relationships with assigned providers and other care management team members.
  • Travel to all CCMSD locations.
  • Connect the patient to community resources or assist in problem solving due to a lack of patient resources.
  • Accepting transfers from the Patient Enrollment team to conduct preliminary health assessments for newly enrolled patients in our network.
  • Resolve patients' questions and create an open dialogue to understand needs.
  • Assist with medication management, including identifying potential medication concerns, adherence, and coordinating refills.
  • Assist in ensuring timely delivery of services to your patients; Home Health, DME, Home Infusion, and other critical needs.
  • Personal Care: Assisting patients with personal hygiene, dressing, bathing, and other daily activities.
  • Health Monitoring: Checking vital signs, setting up medications, and monitoring patients' well-being.
  • Light Housekeeping: Performing basic housekeeping duties like laundry and dishes.
  • Transportation: Providing or arranging transportation for patients
  • Report to charge Care Coordination RN
Education and Experience:
  • High school diploma or equivalent
  • A minimum of two (2) years healthcare experience
  • 1-3 years relevant experience in patient navigation Population specific experience is very important.
  • Experience with Medicare and Medicaid Payers
  • Medical Assistant Certification
  • Certified Nursing Aide Certification in Wyoming
  • Must be 21 years of age.
  • Schedule:
  • 40-hour work week
  • Monday to Friday with some flexibility
  • Remote and on-site

Role Requirements:
  • To work remotely, you must have a strong internet connection, quiet space to take calls and a professional (distraction free) environment in which you can remain HIPPA compliant.
  • Some expectations of travel depend on patient and facility
