Physical Therapist Job Summary:As the Physical Therapist, under the supervision of the HomeCare Rehabilitation Supervisor, you will be a vital member of a dynamic team responsible for organizing and administering prescribed physical therapy programs in an effort to reduce and/or prevent disability and to assist patients in achieving maximum functional potential.Physical Therapist Job Responsibilities:
  • Evaluates and treats patients as prescribed by referring physician.
  • Records treatment, response, progress and discharge information in patient's electronic chart using portable electronic device.
  • Educates patient, family and significant others regarding condition, treatment and home program.
Routinely confers with physician and other practitioners in person, by phone, or in writing, regarding progress, regression or change of plan.Orients, instructs, and directs other Health Care Team members as needed.Physical Therapist Education and Experience Requirements:
  • BS, MS or DPT degree from an accredited program in Physical Therapy required.
  • Connecticut license or Connecticut license eligible required.
  • CPR Certification required.
  • Minimum of one year experience preferred.
  • Valid Connecticut Driver's License and proof of current insurance coverage required.
Physical Therapist Knowledge, Skills, and Abilities:
  • Uses common sense and special medical skills to care for the sick or handicapped.
  • Understands technical information from supervisors, charts, reference books, manuals and labels.
  • Ability to use Portable Electronic Device to record/maintain patient information pertaining to Plan of Care.
  • Works fast and accurately in an emergency.
  • Communicates with people when they are sick, handicapped or nervous.
  • Keeps up to date on trends and developments in the field of physical medicine and health services.
