The focus of this position is to improve Traditional Health Worker (THW) access for Members and increase recruitment and retention of THWs within the community. The THW Liaison will act as a resource for Members and THWs in our service area. This position will collaborate with providers and community stakeholders to improve regional availability of THW services and to support service system improvements. Work includes collaborating with the OHA Traditional Health Worker Commission to implement best practices and meet regulatory requirements. This position will implement EOCCO's THW Integration and Utilization Plan and will be responsible for data collection and submission to the State. The THW Liaison will work collaboratively with other departments involved in the work to coordinate and share progress. This role will conduct internal and external trainings to support the THW workforce. This is a FT Hybrid role based in Portland, OregonPay Range$64,394.27 - $80,498.01, annually. Actual pay is based on qualifications. - 2+ years of experience in healthcare, public health, or community-based organizations, including working in or with rural or underserved communities, working with leadership, facilitating meetings, measuring, and monitoring programs evaluation activities and working with multi‐disciplinary, multi‐cultural teams. 3+ years as a THW or community networking experience strongly preferred.
- Skilled in developing and implementing community partnerships.
- Implement and maintain THW Integration and Utilization Plan in collaboration with key stakeholders.
- Participate in OHA’s Traditional Healthcare Worker Commission and Subcommittees.
- Create and lead a THW Workforce Advisory Committee at the CCO level to advise work activities and include THWs at all levels of process/policy development and implementation.
- Ensure compliance with OHA regulations and the OHA's Traditional Health Worker Commission's best practices.
- Integrate THWs into the delivery of services.
- Provide technical assistance regarding the THW certification process and establishing THW payments.
- Outreach to current and future employers of CHWs.
- Share training and certification processes and payment models.
- Collaborate with other THW Liaison from other CCOs statewide and community partners to build shared annual work plans, deploy trainings and facilitate community coordination that is responsive to stakeholder needs. Maximize opportunities for shared learning.
- Develop THW communication plans to members, providers, and community-based organizations.
- Increase recruitment and retention of THWs.
- Assess barriers to integration and utilization of THWs and their services.
- Assist member with utilizing THW services across all setting.
- Document location and type of all THWs and update regularly. Settings to include clinic, non‐clinic and community-based organizations.
- Identify gaps and mitigation plans.
- Work closely with the analytics department to develop baseline utilization measures and performance indicators over time.
- Work in collaboration with case management and health navigators to understand regional needs of our membership and ensure that THWs are integrated into member’s care teams and part of member’s treatment plans.
- Align work with strategic goals and major initiatives, the Health Council community governance model and the CHA and CHIP.
- Engage the Clinical Advisory Panel and the Community Advisory Council in the THW work.
- Identify barriers that inhibit the ability to meet goals, develop mitigation plans and oversee interventions.
- Create detailed reports of project progress as required by contracts and regulations. Ensure timely submission to OHA of all required deliverables.
- Develop member satisfaction survey with THW services, aggregate results and develop improvement activities.
- Serve as a resource for internal staff, community partners and provider partners on all THW activities
- Represent EOCCO at Oregon Health Authority meetings and conferences. Within Medicaid Quality Improvement, work on meeting the Quality Metrics and implement interventions, as assigned. Document and report quality initiatives to appropriate internal stakeholders.
- Through healthcare research and quantitative analysis, summarize and synthesize complex data and information to identify trends and patterns, pinpoint issues, and areas of improvement, and recommend solutions and next steps.
- Coordinate with marketing staff and vendors as needed to update, create, or purchase relevant materials and communications.
- Work with other departments to develop and vet program materials using guidelines established by OHA and the department.
- Develop annual project work plan resource allocations and budget impacts.
- Oversee data collection and analysis of data and prepares pertinent documentation and reporting for THW activities, including queries and THW, member, and provider surveys.
- Monitor and report the outcome of THW and quality improvement committees, regulatory agencies, and other oversight entities.
- Develop and maintain THW policies and processes.
- Assist with medical and dental health promotion activities to improve the quality and safety of clinical care programs/services provided to EOCCO members.
- Plan and assign quality activities and tasks with a variety of outside entities and a diverse work force, including leadership, operational staff, regulatory, business analysts, technical teams, marketing, and finance.
