- As a Registered Nurse Field Assessor (FA) you would be completing Long Term Care insurance assessments
- These hands-off assessments require you to obtain claimant demographics, physician information, a current medication list, and evaluate the claimant s functional level of independence
- The assessment process also involves completing a brief cognitive screening and evaluating the claimant s ability to complete ADL and IADL tasks
- Provides care as defined within the scope of nursing practice and the Regulated Health Professions Act.
- If directed by the Director of Nursing, makes the initial evaluation visit and regularly reevaluates the client’s nursing needs.
- Initiates the plan of care and makes necessary revisions, keeping the Director of Nurses updated on the revisions.
- Provides those services requiring substantial and specialized nursing skill.
- Initiates appropriate preventive and rehabilitative nursing procedures.
- Prepares clinical and progress notes and summaries of care.
- Informs the Director of Nursing, physician and other personnel of changes in the patient’s condition and needs.
- Counsels the client and family in meeting nursing and related needs.
- Assists in the development of organization goals. Administers agency policies and procedures.
- Assists the Director of Nursing with assessing the competency of clinical staff to promote quality, continuity and safety of a client’s care.
- Supervises, educates and coaches other nursing personnel, as directed by the Director of Nursing.
- Provide information to be used for statistics, reports and records for purposes of evaluation and reporting of organization activities.
- Assures proper maintenance of clinical records in compliance with provincial and federal laws.
- Assures compliance with all provincial and federal laws
- Identifies and participates in agency performance improvement activities.
- Stays informed about changes in the field of nursing and home health care. Shares information with appropriate organization personnel.
- Promotes home health referrals in the health care community.
- Current Certificate of Competence and in good standing with the provincial College of Nurses
- Preferred: Bachelor’s degree in Nursing. Must have graduated from a nursing accredited program.
- Minimum five (5) years of clinical experience with at least half of that time working in home health or related health care organization. Minimum three to five years of direct employee supervision.
- Demonstrated ability to supervise and direct staff.

- Ability to travel on a daily basis including weekends, nights and holidays
- Understands regulatory compliance
- Provincial requirement for the franchise location
- Provincial and federal regulations including but not limited to OSHA, Privacy
- Must demonstrate good clinical reasoning and judgement as it relates to assessing ADL performance, cognition and home safety.
- Must be comfortable with the use point of care technology when completing assessments. Education & Experience
- Current, unencumbered RN licensure in state of practice.
- Minimum of 2 years of home health experience or 2 years nursing experience in an equivalent area of practice serving the older adult population.
- Current driver s license, vehicle insurance, and access to a dependable vehicle.
