Utilizes professional social work skills to assess patients for psycho-social and economic barriers that interfere with the attainment of healthcare goals. Works closely with Case Managers, Physicians, Nursing staff, spiritual support and others, as appropriate, to advocate for the patients and families with counseling services, crisis intervention, stress management, patient and parent coping (as appropriate), bereavement, abuse and neglect issues and continuity of care. Actively participates in outstanding customer service and accepts responsibility for maintaining relationships that are equally respectful to all.The value youll bring to the team:Performs psycho-social assessments on patients and families and provides psychotherapy to patients and families coping with medical issuesMaintains comprehensive community resources and networks to assist patients and families as they transition back to the communityCollaborates with the multi-disciplinary team in establishing/documenting psycho-social determinants to attainment of safe dischargesServes as a consultant and educator to the multi-disciplinary team regarding the psycho-social components of illness, disability and hospitalizationProvides patients with end-of-life support and advance directives; Performs crisis intervention as clinically indicatedFacilitates staff emotional debriefing and ongoing support groupsMaintains professional competency by participating in continuing education and attends pertinent conferences and seminars regarding clinical social workApplies knowledge of local, state, and federal programs to assist with appropriate referrals and coordination of post-acute careParticipates in the development of programs to support patients and families dealing with complicated medical issuesThe expertise and experience youll need to succeed:Minimum qualifications:Master's Degree of Social Work2 years' experience in clinical social workCurrent active State of Florida Licensed Clinical Social Worker, orLicensed Social Worker (LSW)Active American Heart Association BLS1 year of direct clinical with case management experience preferable in a health or mental health setting with an emphasis on mental health, substance abuse and/or geriatric populationPreferred qualifications:Doctorate degreeAccredited Case Manager (ACM), orCertified Case Manager (CCM) This facility is an equal opportunity employer and complies with federal, state and local anti-discrimination laws, regulations and ordinances.
