The Staff Pad is proud to partner with St. Peter’s Health in Helena, Montana, in the search for a dedicated RN Care Manager. This vital position offers the opportunity to influence patient care outcomes and support smooth transitions across the continuum of care within a mission-driven organization.Position Overview:As a CM/UR RN, you will collaborate with patients, families, physicians, and interdisciplinary teams to guide patients through their Emergency Room and acute care journeys. You’ll manage utilization reviews, ensure the appropriate level of care, and create safe and timely discharge plans. This role blends clinical expertise, advocacy, and communication to ensure patients receive the right care at the right time in the most cost-effective way.Key Responsibilities:Access Management / ED Coordination:Partner with medical staff to determine correct admission status and treatment plansMeet with patients/families to assess navigation and transition needsCoordinate reviews and liaise with E.H.R. physician consultantsIdentify and help resolve financial or transition barriersUse high-risk screens to flag complex cases and coordinate with social workProgression of Care:Support adherence to evidence-based clinical protocolsCoach physicians in documentation best practices and complianceJoin interdisciplinary rounds to advocate for effective treatment plansInitiate referrals and manage discharge services such as home health, DME, or hospiceTransition / Discharge Planning:Identify patients with complex psychosocial needs and coordinate social servicesGuide safe discharges or transfers to alternate levels of care, including Swing Bed statusCommunicate plans and transitions clearly to the receiving CM team when applicableRotational Responsibilities Include:Utilization Review: Conduct daily insurance certifications and patient reviewsED Case Management: Assess admission necessity and support outpatient referralsDischarge Planning: Assist inpatient units with discharge logistics and coordinationQualifications:Experience: Minimum 3–5 years of acute care experience requiredEducation: Registered Nurse (RN) required; BSN or equivalent preferredLicensure: Current Montana RN licensePreferred Certifications: Case Management and/or Utilization Review certificationSkills & Attributes:Exceptional communication, negotiation, and time management skillsProficiency in Word, Excel, and familiarity with Meditech preferredAbility to work independently and collaboratively across departmentsLocation:St. Peter’s Health – Helena, MontanaEnjoy the balance of meaningful work in a supportive healthcare system and the natural beauty of Montana’s capital city.
