  • Change Agent Assists others as a change agent and champion for new hospital, ICU related initiatives. Remains readily available to assist others as needed. Able to adjust work load, priorities based on patient need.
  • Compassionate Care Establishes a compassionate environment by providing emotional, psychological, and spiritual support to patients, friends, families, and coworkers.
  • Equipment and Supplies Maintenance Ensures operation of equipment by completing preventive maintenance requirements as per manufactures instruction or calling for repairs when required. Maintains SRN supplies inventory by checking stock to determine inventory level, anticipating needed supplies and placing orders for required inventory.
  • Nursing Practice Standards of Practice Practices nursing according to Franciscan Alliance and nursing division's philosophies and standards of care set by state board of nursing, state nurse practice act, and other governing agency regulations. Evaluates ones own nursing practice in relation to professional practice standards and guidelines, relevant statutes, rules, and regulations.
  • Patient Assessment Assesses, collects, and analyzes comprehensive objective, subjective, psychosocial, and cultural data pertinent to patient condition to determine the nursing diagnosis, patient needs and plan of care. Responds to patient reassessments and responses to care plan. Actively rounds to assess at risk patients or any patients requested by nursing personnel.
  • Patient Education Independence Promotes patient's independence by establishing patient care goals; teaching patient, friends, and family to understand the condition, medications, and self-care skills; answering questions.
  • Plan of Care Development Develops an appropriate plan of care based on objective and subjective data, observations, and communications from patient and family and continuously reviews and revises the plan of care as patient condition warrants.
  • Plan of Care Evaluation Evaluates the individualized plan of care and progress towards attainment of outcomes in collaboration with patient and family utilizing available data resources.
  • Plan of Care Implementation Prioritizes and implements the identified plan, coordinates care delivery, employs interventions to improve patient outcomes while providing a safe, respectful, and ethical practice environment.
  • Safe Work Environment Maintains a clean and safe work environment by handling and clearing instruments, equipment, and supplies in accordance with infection-control policies and protocols. Administers and stores medication according to controlled substance regulations.
  • Stroke Team Membership Participates as an active member of the stroke team. Active Member of Stroke and Chest Pain meetings. Attends other patient care meetings as requested.
  • Highly Dynamic Environment Works in a highly dynamic environment and responds quickly to urgent calls in a calm manner.
  • Code Blue and Rapid Response Events Responds to all Code Blue and Rapid Response events in the hospital and on hospital grounds; directs patient placement.
  • Mock Code Blue Events Participates, leads Mock Code Blue events to maintain staff readiness.
  • Quality Activities Participates in quality activities.
  • Service Program Participation Participates as a leader in service programs for those we are privileged to serve, as a champion for rounding and service recovery as well as has an awareness of coworker satisfaction and satisfaction of those we are privileged to serve scores. Attends and participates at in unit and SRN meetings.
  • Collaboration and Patient Experience Collaborates with patients and their families, and or primary caregiver as well as members of a multidisciplinary team, to ensure optimal patient outcomes and enhancement of the patient experience.
  • Patient Assessment Ongoing Reassessment Communication Communicates timely changes in patient assessment, ongoing reassessment to providers that warrant consideration and or a change in the plan of care and maintains and coordinates communication with all members of the multidisciplinary team to facilitate safe transitions and continuity in care delivery. Maintains continuity among nursing teams by documenting the assessment, plan of care, interventions, and responses to care in the electronic medical record in a timely manner.
QUALIFICATIONS
  • Required- Associates/RN Diploma
  • Preferred- Bachelor's Degree
  • Required- 2 years Critical care experience
  • Preferred- 1 year Charge nurse or supervisor experience
  • Required- Cardiopulmonary Resuscitation (CPR)
  • Required- Registered Nurse (RN) License
  • Required- Advanced Cardiovascular Life Support (ACLS) - American Heart Association
  • Required within 180 days- Pediatric Advanced Life Support (PALS)
  • Required within 180 days- Stroke Certified - American Board of Neuroscience Nursing (ABNN)
