Through our multi-site network of care, we constantly strive to meet its mission in helping people find their way while educating and empowering individuals to participate in their own care and recovery.Job SummaryThe Case Manager is responsible for providing a range of services for clients with either Psychiatric and/or Substance Abuse issues. Also provides advocacy, outreach, and service linkage for clients and initiates access to entitlement services.Qualifications
- Experience working with a behavioral health and/or chemical dependency population
- Bachelor's degree in Human Services or related field, Social Work preferred
- Valid driver’s license may be required based on program
- Knowledge of community resources preferred.
- Computer proficiency and keyboarding skills required
