
Transfer and Access Navigator

- Requests and/or provides information regarding the transport of ill or injured patients to and from a Brown University Health’s institution. Serves as coordinator for the admissions and transfers of said patients
- Orchestrates patient movement throughout Brown University Health’s system through algorithms and guidelines with regard to balancing bed supply and demand while also focusing on patient’s needs.
- Provides monitored communication links among persons necessary for consultation, referral and transport of patients utilizing state of the art communications equipment.
- Maintains communication with transport team and tracks estimated times of arrival and departures maintaining strong communication and coordination with receiving site.
- Collaborates with internal physicians (system-wide) to build workflows and best practice guidelines to assist with coordinating transfers appropriately.
- Completes all required documentation including records into the EMR with emphasis on clarity, efficiency, and completeness within appropriate time frame.
- Coordinates demographic and insurance company information with Admitting Departments and case manager across Brown Univ Health on repatriation processes and agreements.
- Documents all critical care transports within the EMR giving ability to collect statistical data on but not limited to: transport volumes, transport types, and demographic information.
- Works with Capacity and Flow team members including capacity and flow navigators to understand bed availability across the system and any concerns or issues that would impact a site’s ability to accept a patient.
- Coordinates appropriate and efficient placement of patients guided by protocols, algorithms, and standard operation procedures and working with the Admitting Department and clinical units.
- Appropriately shares and directs pertinent information with EpiCenter Leadership as needed.
- Assures that all communication and support equipment is properly functioning.
- Participates in community outreach and in-house educational efforts, such as nursing grand rounds etc.
- Works with appropriate medical service line to follow up with referring facilities regarding patient outcomes and concerns related to the transfer process.
- In conjunction with -the critical care transport team, follows up with referring facilities regarding any concerns with requests for transport.
- Meets and maintains mandatory/safety education and clinical competencies measured by pre-determined credentialing standards.
- Maintains collaborative relationships with team members, other members of the health care system and outside agencies including but not limited to: transferring and receiving hospital customers, EMS personnel and community agency representatives.
- Participates in and when appropriate leads teams meetings such as capacity huddles, and surge calls.
- Participates in Quality Assurance, both in meetings organized for that purpose, and on a daily basis using established quality indicators for department.
- Participates in the orientation of new employees as assigned.

- Maintains data and reports related to patient safety, quality, and throughput
- Demonstrates innovative thinking with ability to think through workflows and processes and recommend strategies for improvement.
- Demonstrates professional communication skills (verbal and written)
- Must have current licensure to practice as a Registered Nurse in the State of Rhode Island or in another state that participates in the Nurse Licensure Compact. Baccalaureate in nursing preferred. Must complete credentialing specific to Transfer & Access center within 3 months.
- Three to Five years’ current experience as a professional nurse providing care within an intensive care unit or emergency department is required. Critical Care Transport experience preferred.
- Strong computer skills with knowledge of EHR (Epic), computer programs such as excel, outlook, PowerPoint, access, and word, which will be used for retrieval of demographic information, performance data and statistics.
- Ability to operate transmission equipment.
- Highly effective verbal and written communication skills are required to work with transport staff, physicians, and other hospital personnel and patients.
- Ability to demonstrate effective customer service behaviors.
- Visual and aural ability to work with computer and transmission equipment.
- Physical requirements include the ability to sit, for long periods.
