UPMC Home Healthcare and Family Hospice is hiring a Home Care Aide to support Erie County. This is a Monday through Friday, 8:00 AM-4:30 PM position with rotating weekend shifts, providing exceptional care to patients in their place of residence. This is a field position, making home visits to both home health and hospice patients.Responsibilities:
  • Communicates appropriately with the interdisciplinary home care team promptly, reporting any changes in the patient's condition to the appropriate professional staff.
  • Attends compliance training and adheres to the organization's standards of conduct, policies, and procedures.
  • Focuses on customer service and continually strives to perform the duties of their job in a manner that will result in optimal patient satisfaction.
  • Provides safe and direct personal care to patients in the home setting under the direct supervision of appropriate professionals.
  • Performs duties and job responsibilities in a fashion that coincides with the service management philosophy of the organization towards patients, visitors, staff, peers, physicians, and other departments within the organization.
  • Identifies, develops, and participates in process improvement opportunities within the home health agency that will enhance the quality of services we provide.
  • Proven ability to facilitate change.
  • Maintains and completes documentation accurately and promptly while in the patient's home and within the agency and regulatory standards.
  • Participates in all required agency meetings.
  • Must maintain annual competency evaluation scores as determined by regulatory requirements

  • High School Diploma or GED - Required
  • One (1) year of experience - Preferred
  • Certified Nurse Aide - Preferred
  • Minimum of 75 hours of Nurse Aide training - Required. If no previous nurse aide training, applicant may attend UPMC Home Healthcare Nurse Aide Training Course as available upon hire.
Licensure, Certifications, and Clearances: * CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire
