- Provides safe efficient age specific nursing care to patients/families using the nursing process in order to assist them to attain, maintain, regain, and sustain optimal health status.
- Functions in accordance with the professional standards of the Ohio board of nursing.
- Completes/submits required documentation.
- Delivers effective relationship based nursing care.
- Demonstrates technical skills in care delivery that result in safe practice and positive care outcomes.
- Demonstrates clear communication and effective collaboration to optimize patient care and clinical operations.
- Develops self and contributes to the professional practice of nursing.
- Displays leadership that influences optimal clinical and operational outcomes.
- Performs other duties as assigned.
- Complies with all policies and standards.
- For specific duties and responsibilities, refer to documentation provided by the department during orientation.
- Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients.
- Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace.
- 100% travel to the patient's homes for care.
- This is the day shift.
- Monday-Friday with rotating weekend and holidays.
- This LPN will provide coverage for Geauga/Western Ashtabula Counties.
- Other Accredited Program: Completion of accredited practical nurse program or currently enrolled in a nursing program required.
- 2+ years Nursing Experience (Level II only) (Required).
- Knowledge of LPN scope of practice according to the Ohio board of nursing.
- Uses a wide variety of technical equipment to meet patient needs and prescribed clinical therapies (i.e. stethoscopes, thermometers, sphygmomanometers and electronic monitoring devices).
- Level II Only: 18 CNE's per year. • 3 CNE's in clinical specialty • At least 5 CNE's in Leadership • Other CNE requirements will be agreed upon with manager
- Level II Only: • 3 Peer Reviews. • Committee Involvement. • 5 criteria with evidence of each.
- Knowledge of LPN scope of practice according to the Ohio board of nursing.
- Uses a wide variety of technical equipment to meet patient needs and prescribed clinical therapies (i.e. stethoscopes, thermometers, sphygmomanometers and electronic monitoring devices).
- Licensed Practical Nurse (LPN), Ohio and/or Multi State Compact License Required Upon Hire.
- Basic Life Support (BLS) Required Upon Hire.
