AEDD has an immediate opening for a Direct Care Supervisor. Requires a bachelor's degree in the field of human service or Bachelor's degree from four-year college; or university plus two years experience as a Case Manager working with individuals with the developmentally disabilities; or four years' experience as a Case manager in a related field may be used in lieu of education. Position will supervise. Immediate hire requires valid driver's license & proof of auto liability insurance. Criminal background checks and a drug screen will be conducted. EOE/M/FStarting Salary: $35,658 AnnuallyJob Summary: The Direct Care Supervisor for ACS works under the direct supervision of the Assistant Director of ACS and has direct supervision over assigned Direct Support Professionals. The position initiates and maintains the referral/intake/admissions process, assists guardians with the admissions and service process, insures that Medicaid eligibility of all consumers are kept current, serves as the liaison to the billing, Adult Clinical Services, and nursing/mental health departments, assists in the creation and maintenance of accurate and current admissions records, develops consumer service plans, assists with the establishment and maintenance of all client files and assists certified staff with coordination of client services.Essential Duties and Responsibilities include the following.   • Serve on Inter-disciplinary Admission Committee, duties include: attend Person Centered Service Plan meetings with individual and guardian, assist in completion of admission forms and assist in initial habilitation program and establishment of goals.
  • Complete forms on all individuals as required by Arkansas Social Services, DDS, Social Security Administration, and any other agency providing service to the individual - Medicaid eligibility, Social Security, Representative Payee etc.
  • Responsible for hiring appropriate number of staff to provide direct care to consumers on case load.
  • Maintain records on all individuals served as required by DDS, including: initial application, permission forms, habilitation plan, staffing reports, progress reports, quarterly reports, behavior incident reports, and medical information
  • Ensures services are provided to consumers as defined in the Plan of Care. Contacts consumers and guardians, as required to insure consistency in programming.
  • Serve on Multi-Agency Team, which is responsible for staffing individuals progress in programs and insuring changes in program is made when necessary and be in constant communication with Case Management Agency.
  • Be available and responsible to all individuals we serve as well as caregivers.
  • Ensure Direct Support Professionals and following consumer plan of care and applicable company policy in order to ensure quality of care being given.
  • Assist in serving as liaison with other services to insure coordination of services to individuals, such as: utility assistance, food stamps, HUD housing, etc.
  • Monitor and maintain AEDD Owned and Operated Homes, ensuring routine maintenance needs are met timely by conducting monthly inspections and ensuring that the house manager turns in bi-weekly physical plant safety forms as well as taking any direct needs from the staff at each location.
  • Make Quarterly visits to homes, announced and unannounced.
  • Maintain and update files with tenant's leases with AEDD for the group homes.
Supervisory Responsibilities: Directly supervises Direct Support Professionals assigned to consumers on case load. Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities include interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems.Qualifications: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
