Job Description:Job OverviewThis position is located at Maple Grove and Brooklyn Park.OverviewAs a member of the primary care team, the Behavioral Health Clinician assists in the team's management of the overall health care needs of the enrolled patient population. The Behavioral Health Clinician's primary functions include assessment, consultation and direct care related to the prevention, identification, treatment, and ongoing management of psychosocial/behavioral concerns which impact the overall health of the defined patient population.Responsibilities
  • Conducts assessment and evaluation of behavioral health conditions by means of diagnostic and screening tools, clinical interviews, and consultation with the medical team.
  • Conducts diagnostic evaluation and documents assessment in medical record.
  • Conducts diagnostic evaluation and confers diagnosis as appropriate.
  • Develops an individualized treatment plan of care.
  • Collaborates with patient, family, and other colleagues to develop a treatment plan of care.
  • Develops an individualized plan based on age, social and cultural considerations, condition, and other relevant needs.
  • Maintains required, appropriate, and timely communication and correspondence with treatment authorization and payer sources in a manner which promotes efficient and effective delivery of clinical services.
  • Contributes to the development and implementation of treatment plan in collaboration with the patient and primary care team.
  • Provides consultation to the medical team aimed at the prevention, identification, treatment and management of behavioral health/psychosocial issues and conditions.
  • Functions as an effective team member as evidenced by working in a collaborative approach with primary care providers.
  • Demonstrates basic knowledge of psychotropic medications.
  • Understanding of integrative health care in terms of the interactive and interdependent expressions of the physical/biological, emotional, and psychological dimensions of health.
  • Employs a primary care approach in addressing the behavioral/psychosocial aspects of routine and complex medical conditions.
  • Provides primary behavioral health interventions for identified mental health and substance use conditions.
  • Delivery of population-based health care by providing individual, group and community-based services.
  • Knowledge base and clinical skills in applying evidence based behavioral interventions for identified medical conditions.
  • Maintains concise, timely and accurate documentation in the patient’s medical record.
  • Connects patients with community resources as needed to meet basic living and support needs.
  • Effectively refers and manages collaboration and communication across the continuum of care.
  • Demonstrates ability to provide care or service adjusting approaches to reflect developmental level and cultural differences of population served.

  • Required to complete system credentialing to perform pro fee billing

Required Qualifications
  • Master’s or doctoral level degree in counseling, social work or psychology
  • 2 years experience post licensure
  • One of the following licenses
  • Licensed Marriage and Family Therapist (LMFT)
  • Licensed Professional Clinical Counselor (LPCC) or
  • Licensed Independent Clinical Social Worker (LICSW) or
  • Licensed Psychologist (LP)
  • Licensed Graduate in Social Work will be considered under certain circumstances
Preferred Qualifications
  • 5 years experience post licensure
