Position SummaryThe Designated Coordinator (DC) is a hybrid office-based/field-based role overseeing the delivery of Home and Community-Based Services (HCBS) for individuals enrolled in New Wave Home Health Care's 245D program. The Designated Coordinator will demonstrate New Wave’s core values at all times including empathy, integrity, and collaboration while modeling a supportive environment for clients, families, and staff.ResponsibilitiesThe Designated Coordinator is responsible for ensuring the highest quality person-centered services and achieving desired client outcomes in compliance with Minnesota Statute Chapter 245D.081 requirements. Key responsibilities of the Designated Coordinator position include:· Coordinating services based on client assessment and care plans while ensuring compliance with Minnesota's 245D requirements.· Providing leadership, support and supervision to Direct Support Professionals (DSPs), including annual DHS staff training.· Developing and implementing individualized, person-centered program plans using positive support principles, required 245D paperwork such as CSSP and IAPP, and other critical documentation.· Collaboration with families, case managers, and several interdisciplinary care teams to advocate for the patient, including scheduling and attending regularly required progress evaluation meetings.· Updating progress evaluation reports to align with person centered goals and desired outcomes achieved through program.· Advocating for and coordinating personal care resources and support services in the persons’ life where assistance is needed including medical visits and transportation.· Monitoring service delivery and working with staff to ensure the persons’ needs are met according to annual service plan.· Building meaningful and respectful relationships with New Wave clients, their families and their community touch points with appropriate boundaries.· Able to educate others on eligibility for different DHS waiver programs and services in the State of Minnesota.· Ensure timely completion and review of service agreement plans and assessments while maintaining accurate documentation and compliance with MN state requirements.· Maintaining positive relationships with all external partners while serving as a primary point of contact for clients, staff, as well as County and State case management representatives.· The Designated Coordinator may also need to respond to client and staff calls in case of emergencies.
  • Candidate must have general office skills including answering phones, customer contact etiquette, familiarity with Microsoft Office, filing systems and database system with training.
State Required QualificationsCandidates must meet the qualifications for a Designated Coordinator as outlined in Minnesota Statute Chapter 245D.081, which includes various combinations of education and experience in human services, direct care, or related fields. Required qualifications include:· A bachelor's degree in a field related to Human Services and one year of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older; OR· An associate's degree in a field related to Human Services and two years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older; OR· A diploma in a field related to Human Services from an accredited postsecondary institution and three years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older; OR· A minimum of 50 hours of education and training related to Human Services and disabilities and four years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older under the supervision of a staff person who meets the state qualifications.Other Qualifications· Proven ability to consolidate case information to develop person-centered support plans.· Strong knowledge of 245D HCBS licensing standards and MN 245D Statutes· Excellent organizational, multi-tasking and project management skills.· Ability to work independently in an office, and in the field, while managing multiple tasks related to project management and compliance needs.· A valid driver's license with an acceptable driving record and proper insurance· CPR/First Aid certification (or willingness to obtain safety training as needed).· Ability to complete State mandated annual training modules as assigned to maintain certification.· Ability to clearly communicate to staff and clients at a professional level, including ability to speak, read and write English.· Preferred Experience: When applying please highlight any office experience using technology including, Microsoft Office 365 (Word/Excel/Powerpoint/Teams), Google Suite, Reviving Care, Customer Relationship Management Systems, Phone Apps, Etc.To ApplyTo apply, please complete an online or in-person application and include your resume with current contact information.
