Internal Applicants: Apply via email, following the emailed application instructionsPosition Summary: The Pharmacist provides treatment adherence counseling to clients enrolled in case management services. The goal of treatment adherence counseling is to ensure readiness for, and adherence to, complex HIV/AIDS treatments. This enhanced treatment adherence counseling service, includes the following key activities: (1) initial in-depth evaluation and assessment of barriers to and reasons for non-adherence; (2) development of a comprehensive, individualized Treatment Adherence Plan; (3) treatment adherence counseling interventions; (4) ongoing client monitoring to assess the efficacy of the Treatment Adherence Plan; and (5) periodic re-evaluation and adaptation of the Treatment Adherence Plan as necessary. This includes all types of counseling interventions (both individual and group) including face-to-face, phone contact, and any other forms of communication. The pharmacist identifies and actively works to remove patient barriers to adherence. Services are designed to be low barrier and meet the needs of vulnerable clients.The Pharmacy team at HIV Alliance includes two clinical pharmacists and a Pharmacy Assistant who assists the Pharmacists with administrative duties.Essential Functions and Responsibilities:
- Client Management: Carry an active caseload of eligible clients, providing direct one-on-one pharmacy adherence services. Able to provide services in-person, over the phone and on a drop-in basis.
- Assessments & Planning: Conduct treatment adherence assessments and goal planning in collaboration with the client to identify medication adherence plans and barriers to care. Develop individualized care plans based on assessments and available resources to improve medication adherence.
- Coordination of Services: Work collaboratively with case management team, providers, community partners to facilitate access to HIV treatment and medical care. Provide client education pertaining to HIV and other medications, disease progression, and risk reduction. Perform drug interaction checks on client medication lists. Review medication list for appropriateness of therapy.
- Advocacy: Advocate for clients with medical service providers and case managers.
- Documentation: Maintain accurate and timely documentation of client interactions, placements, services provided, and progress updates in agency databases.
- Referrals: Refer clients to medical resources as needed and follow up.
- Community Engagement: Build partnerships with medical providers in our service region. Attend relevant community meetings and participate in ongoing training and professional development.
- Program Education: Develop and conduct staff and client education on HIV medication, treatment, disease progression and relevant therapies.
- Standards: Comply with all applicable program standards, guidelines, laws and administrative rules. Other tasks as assigned, including training/cross-training other staff as needed.
- Reporting: Assist management with all necessary reports.
- Valid Oregon Pharmacy license
- One year of experience providing clinical care to patients with chronic disease or infectious disease
- Certification and/or experience in HIV specialty care, or ability to obtain certification such as AAHIVP through the American Academy of HIV Medicine, as providing by the agency
- TB test (provided at agency)
- Valid Oregon driver's license, or valid out of state driver's license with the ability to get an Oregon driver's license within 30 days of establishing residency in Oregon, and a driving record sufficient to be covered by agency auto insurance policy.
- Ability to pass agency criminal background check

- Bend, stand, & reach to be able to transport clients and to lift and carry up to 25 pounds for tasks such as building kits & assembling testing/educational materials.
- Use continuous clarity of vision at 20 inches or less for processing of paperwork
- Ability to work flexible hours, including early mornings, evenings, and weekends
- Ability to work 5 days per week for a minimum of 40 hours per week
