Description :The Health Home targets populations who are in Shepard Pratt’s day programs, Assertive Community Treatment (ACT) program, or residential rehabilitation programs. The team provides an extra layer of care that treats the whole person by identifying gaps and barriers to treatment.A more comprehensive description of Sheppard Pratt’s behavioral Health Home services, including the core services that it offers, can be found .What to expect.This is an opportunity to coordinate somatic healthcare in tandem with mental illness, emotional disturbance, and/or substance use disorder treatment. You will provide advisory and advocacy for program staff and clients.Specific responsibilities include:
  • Participating in initial treatment planning and development of associated goals.
  • Ensuring clients have access to medical providers and hospitals, including coordination of admission and discharge.
  • Ensuring clients complete all required assessments, screenings, and immunizations.
  • Training staff on medical diseases, treatment, and medication.

Pay for this position is determined on a number of factors, including but not limited to, years and level of related experience.What we need from you.
  • Must be a Maryland Registered Nurse at time of application.
  • 1-3 years of nursing experience.
  • A driver’s license with 3-points or less and access to an insured vehicle.
