- Actively monitors and amends patient visit schedule to maximize productivity and patient need. Takes action to account for new patients and changes in existing patient schedule, including but not limited to, calling patient to reschedule visits, reshuffling patient load to meet new demands, communicating recommended changes to supervisor, etc.
- Performs a comprehensive (OASIS and SLP) assessments of assigned patients including, but not limited to psychosocial, educational, safety and risk factor parameters, as evidenced by clinical documentation.
- Develops a plan of care to promote optimal functional independence and safety as documented by clinical record.
- Plans for and provides skilled speech-language therapy treatment including, but not limited to, therapeutic techniques, patient/care giver education, etc. assessing the patient's status and progress on each visit and ensuring quality of care as documented in the clinical record.
- As Care Manager and/or team member, coordinates patient care with other clinical team members, including but not limited to, physicians, nurses, care givers, other rehab services and students, ensuring optimal patient care and communication as noted in the clinical record.
- Accurately and completely performs functions as related to school-based services, including, but not limited to, evaluations, documentation, development of Individual Educational Plan IEP's, participation in meetings, and provision of direct and consultative services. Fills out and completes insurance paperwork.
- As necessary, assesses patients' needs for HCA services, develops and coordinates POC with patient/care giver, and provides on-going supervision.
- Coordinates safe patient discharges and/or transfers (or upon knowledge of) and ensures all appropriate paperwork and patient information is processed.
- Serve as a Home Care ST as required
- Master's degree in Speech Language Pathology required
- Current Vermont State SLP license required
- Two years of clinical Speech Language Pathology experience required
- Active enrollment in clinical fellowship year or successful completion of certificate of clinical competency required
- Valid Driver's License
