- Evaluation of resident needs, conditions, care and assist in developing nursing care plans for individual residents, including rehabilitative activities and instruction in self-help
- Communication with family members regarding the condition and progress of the resident including in-person discussion and answering of telephone inquiries
- Requisition of the necessary supplies, equipment, emergency drugs, and other materials to meet the facilities needs. Monitors equipment condition, supplies, etc and reports needs to DON
- Maintains facility records, including charting duties and maintenance of narcotic records
- Arranges the schedules, appointments, and transportation necessary for residents in connection with diagnostic, therapeutic, dental, or medical services
- Responsible for the admission, discharge, or transfer of residents
- Responsible for work assignment of individuals in the nursing area with authority to make changes and revise duties in order to meet resident needs. Is responsible to ensure that staff members complete assigned duties
- Where necessary, the Charge Nurse (LPN) is to provide counseling to employees on performance improvement
- Assist the DON with implementation of policies and procedures
- Review treatment sheets and other charting for accuracy and completion
- Assure implementation and compliance of nursing staff with facility policies and procedures to include, but not limited to, HIPPA, Infection Control, Material Safety Data Sheets, Incident Reporting and Quality Assurance
- Recognize and interpret symptoms and report resident’s condition
- Complete filing of records as assigned with total access to charts
- On weekends, will be required to retrieve and distribute mail to residents and any other time deemed necessary
- Functions as an in-house expert regarding standards of practice, Federal and State regulations and laws, and professional code of ethics governing nursing services.
- Assumes accountability for information in personnel policies and procedures
- Maintains strict confidentiality of resident, employee and facility information
- Consistently works with residents, families, staff, consultants, physicians and ancillary service providers in a cooperative manner
- Implements admission, transfer and discharge procedures; coordinates with other departments, physicians, and ancillary service providers and documents in accord with facility policy and regulations
- Follows established standards of nursing practice and performs nursing skills knowledgeably and competently
- Supervises delivery of meals and proper diet, distribution of nourishment, application of postural supports and restraints
- Accepts responsibility for the safety of residents under his / her supervision
- Assists nursing assistants with resident care as necessary and required by circumstances
- Receives, investigates and resolves or reports to higher authority concerns from residents and their families in accordance with positive customer relations principles. Listens to resident, family or physician concerns and initiates corrective action as necessary
- Assumes responsibility for unit or shift compliance with facility policy and procedure, rules, regulations and standards of practice. Assures that residents receive needed nursing care and service to meet their needs according to the established plans of care and physician’s orders and instruction
- Interprets facility policy and procedures, rules and regulations to subordinate nursing staff, residents, families and appropriate medical and ancillary personnel
- Provides corrective instruction to staff concerning quality of care, quality of life, customer service, and positive image issues
- Monitors and evaluates staff work behavior and performance of assigned duties, takes corrective counseling and disciplinary action according to established policy and procedures
- Identifies staff training needs
- Provides input to the performance appraisal process
- Other duties as assigned by DON or Administrator
- Receives, transcribes, implements physicians orders
- Makes rounds with the physicians, reviewing the physicians’ orders, ordering medications, maintenance of the residents’ charts and transcription of physicians’ orders
- On weekends, will be required to retrieve and distribute mail to residents and any other time deemed necessary
- Observes, evaluates, and documents resident condition or changes in resident condition
- Documents resident care provided and resident’s response or lack of response to care provided
- Administers and documents, according to the standards of practice, medications, enteral / parenteral nutrition, treatment as prescribed by the physician, administers PRN medication and treatments
- Accurately performs and documents comprehensive assessments, or parts of the comprehensive assessments, as assigned
- Plans daily work assignments and schedules based on residents’ needs and available resources, effectively communicating to staff and monitoring staff compliance with assignments and schedules
- Team members and Supervisors: must trust in the organization and its leaders that they will be put into situations that aligned with the culture of Happy Siesta.
