- Perform patient and family initial palliative care consultations, including comprehensive assessment of pain and other symptoms
- Perform or facilitate follow-up visits for palliative care patients as appropriate to facilitate coordination and continuity of care
- Evaluates the physical and psychological state of the Palliative Care patient. Provides support to patients by participating in patient and/or family teaching, counseling and education as well as referring patients to other Fred Hutch services and community agencies to promote optimal adjustment and functioning
- Provide services related to palliative care and end of life care
- Prescribe medications including controlled substances to the extent delegated and licensed.
- Collaborates and consults with a multidisciplinary team to ensure safe, effective, and timely treatment as well as continuity of care.
- Communicates changes in patient’s health status or urgent patient issues to the patient’s primary oncology or hematology care team in a timely manner.
- Documents patient care in accordance with Fred Hutch & UWP policies
- Facilitate continuity of communication and coordination of care with Fred Hutch and community providers as necessary
- Arrange and/or attend and actively participate in palliative care consultation meetings with attending and other palliative care staff in collaboration with treating medical team.
- Participate actively in palliative care service line activities, including but not limited to quality improvement projects, interdisciplinary team rounds, education
- Education of Fellows, Attendings, Advanced Practice Providers, nurses and other clinical staff regarding palliative care through informal and formal means (including rotations on the PC service)
- Fred Hutch APPs may be required to participate as a rotating member of the clinic’s Rapid Response/Code Team.
- Teach and mentor new APP staff & PA and/or NP students
- Willingness and desire to participate in palliative care related research
- While not a component of the current job, willingness to consider weekend or evening call as well as inpatient PC consultations if deemed necessary in the future
- Desire to practice in an academic environment at a Comprehensive Cancer Center
- Other duties as assigned
- Strong team player with a can-do attitude is a must! Ability to work as part of a multi-disciplinary team is essential.
- Graduate of an accredited Physician Assistant or Nurse Practitioner program required
- Washington state & DEA licensure are required
- ACLS required within 3 months of hire
- Strong oral and written communication skills required
- At least one year of ARNP or PA-C experience in Palliative Care; Medical Oncology experience desired
- Current certification in Palliative Care
- Trained in Vital Talk methods and completed the Vital Talk “Train the Trainer” Course
- Conducted or participated in palliative care research
- Graduated from a Palliative Care APP fellowship or experience in the training of palliative care APPs
- Experience with pain and symptom management
- Prior experience with EPIC preferred
