OverviewGentiva Hospice is looking for an Admissions Coordinator. Key Responsibilities:
Serve as the primary point of contact for all new patient referrals
Initiate outreach to patients and families within 1 hour of referral
Coordinate all admissions within the company’s 3-hour commitment window
Verify insurance benefits, payer source, and secure authorizations for hospice care
Communicate pending admissions to appropriate clinical and admission staff.
Collaborate with clinical teams, billing coordinators, and external referral sources
Enter accurate patient data and documentation into the electronic system
Participate in morning stand-up meetings, marketing activities, and outreach including seeking new referrals sources and developing strategies for company growth
Maintain communication with pending and Not Taken Under Care (NTUC) patients
Support weekend and on-call admissions as needed
