This role involves working closely with patients in long-term care (LTC), memory care, and outpatient facilities.Key Responsibilities:
- Provide direct patient care under the supervision of RNs or physicians.
- Conduct health coaching sessions to promote wellness and chronic condition management.
- Perform accurate medical documentation in compliance with healthcare regulations.
- Assist in outpatient settings, LTC, and hospital medicine environments.
- Administer medications and treatments as prescribed.
- Monitor patients’ vital signs and report any changes.
- Support patients undergoing dialysis and educate them about the process.
- Utilize medical terminology to communicate effectively with healthcare teams.
- Conduct diagnostic evaluations to assess patients’ health status.
- Ensure infection control protocols and maintain a safe patient environment.
- Active LPN License in the state of practice in Massachusetts or a pending application for Massachusetts.
- Minimum 2 years of Long-Term Care (LTC) experience.
- Must be able to adapt quickly and work with minimal orientation.
- Experience working in memory care settings preferred.
- Excellent communication skills – verbal and written.
- Ability to work collaboratively while maintaining patient confidentiality.
- Flexibility to adapt to varied healthcare environments and schedules.
