RN – Case Manager - RNCM 25

• Perform case and resource management activities to support optimal patient outcomes
• Conduct utilization management reviews to ensure appropriate level of care and services
• Collaborate with physicians, nurses, and interdisciplinary teams to develop individualized patient care plans
• Coordinate discharge planning and continuity of care across healthcare settings
• Document all case management activities in compliance with hospital and regulatory standards
• Advocate for patient needs while balancing cost-effective care delivery
• Participate in quality improvement initiatives to enhance case management practices

• Minimum 2 years of experience in case/resource management and utilization management (required)
• Strong knowledge of discharge planning, utilization review, and care coordination processes
• Excellent communication, critical thinking, and problem-solving skills
• Ability to work collaboratively within a multidisciplinary healthcare team
