
Clinical Research Professional I / II, COM, Internal Medicine, Division of Infectious Diseases

- Conduct initial and ongoing assessments of patient needs, including social determinants of health.
- Develop and implement individualized, patient-centered resource/care plans.
- Coordinate referrals and follow-up for medical, mental health, substance use, housing, food, and transportation services.
- Monitor patient progress monthly and adjust care plans as needed.
- Assist with insurance applications, medication access, and benefit enrollment.
- Provide education on HIV, medication adherence, safer sex practices, and health literacy.
- Conduct off-site visits on occasion, attend patient appointments, and provide informal counseling and advocacy.
- Identify and engage individuals living with HIV and high-risk populations.
- Build and maintain relationships with community-based organizations and medical homes.
- Participate in outreach events, support groups, and community education sessions.
- Represent the agency at conferences, summits, and professional trainings.
- Maintain accurate and timely documentation in case notes, assessments, and databases.
- Ensure compliance with funding requirements and agency protocols.
- Participate in quality assurance, program evaluation, and internal audits.
- Work closely with interdisciplinary teams including social workers, clinicians, and public health partners.
- Attend staff meetings, supervision sessions, and collaborative planning activities.
- Support agency-run events and contribute to a positive team environment.
Bachelor’s Degree in a related field.
Required Experience
Minimum one (1) year relevant experience.
Five (5) years experience and/or other specialized training can fulfill minimum education and experience requirements.-OR-Clinical Research Professional II:Required Education
