• Duration: 13-week nursing contract
• Shift: 8:00 AM – 4:00 PM, 8-hour day shifts
• Hours per Week: 40 (Guaranteed 37.5)
• On-call: Required, including overnights and weekends (1-2 per month)
• Estimated Weekly Salary: $1980 - $2120

• Active New Jersey RN license – required for all New Jersey nursing jobs
• Minimum 3 years of experience in a hospice care facility (not long-term care)
• Previous hospice travel RN or locum tenens nursing experience required
• 2-3 years as a Registered Nurse in a home hospice environment
• BLS certification (required for healthcare professionals)
• Valid New Jersey driver’s license
• Proficiency with EMR systems; remote EPIC experience strongly preferred
• Excellent clinical assessment, critical thinking, and communication skills

• Perform comprehensive patient assessments to determine hospice eligibility and develop individualized care plans
• Ensure effective utilization of hospice services and maintain visit productivity standards
• Accurately complete admission, discharge, and revisit documentation in EMR systems
• Educate patients and families using age-appropriate methods; document all education provided
• Manage case load, including plan of care development, implementation, and evaluation
• Participate in interdisciplinary team (IDT) meetings every two weeks and communicate patient progress
• Maintain a safe environment, adhere to infection control protocols, and manage medications safely
• Attend monthly staff meetings and contribute to performance improvement initiatives
• Mentor new staff and cross-train to support agency growth
• Provide on-call coverage as scheduled, including overnight and weekend shifts
