Hospice Liaison Nurse

- Education: High School Diploma or GED, required. 4 Year / Bachelors Degree, preferred. Graduate of an accredited school of nursing required; BSN preferred.
- Experience: Minimum three years' experience in home care or hospice, required. Previous experience in managed care, infusion care, and case management and previous home health/hospice case management experience, preferred.
- Licensure/Certification: Current RN licensure in appropriate state, required.
- Additional Skills (required):
- Excellent nursing assessment skills/judgment and understanding of Hospice philosophy.
- Knowledge of Hospice services necessary to obtain and interpret information in terms of the patient’s needs in order to appropriately receive referrals.
- Knowledge of Medicare/Medicaid rules/regulations necessary to understand criteria for determining patient eligibility for admission.
- Knowledge of nursing and pharmacy procedures and standards of practice, both general and specific, to the specialties of infusion and enteral therapies.
- Strong writing skills, organizational and analytical skills.
- Must be flexible, adaptable to change and able to apply problem-solving skills and critical thinking skills.
- Ability to interact effectively with diverse individuals at all levels of authority, in teams and on committees.
- Must have computer experience and have reliable transportation to and from all work related assignments and activities.
- Must be flexible to travel as needed between different locations, as assigned, in all weather conditions and on roads in varying degrees of repair.
- Ability to work in environment containing distractions and interruptions such as noise and telephone calls.
- Ability to handle varying and unpredictable situation such as emergencies or crisis situations.
- Must be available for after hours call on rotational basis.
- Specific vision abilities required by this job include close vision, distance vision, night driving, and ability to adjust and focus.
