This position will support our C-YES Program. This is a full-time, fully remote position that can be located anywhere in the state of New York.About Children & Youth Evaluation ServicesThe Children and Youth Evaluation Service (C-YES) is a New York State Program that determines Home and Community Based Service/Level of Care (HCBS/LOC) eligibility for youth with medical, behavioral, and/or developmental needs aged 0-21 years. The C-YES program also provides Medicaid application assistance services to children who appear HCBS eligible and are either without Medicaid and/or have Medicaid but opt out of Health Home (HH) Care Management. C-YES also develops person-centered Plans of Care (POCs) and provides ongoing HCBS service coordination for those families who elect to remain with C-YES and opt out of more comprehensive case management provided by Health Home Care Management Staff. HCBS are meant to help youths remain in their community as opposed to going to a higher level of care (i.e., facility or institution). A Care Coordination team comprised of Nurse Evaluators and Family Support Coordinators are responsible for ongoing support to all families referred to C-YES.Essential Duties and Responsibilities:- Responsible for hiring, training, supervising and developing of Nursing Staff
- Monitor the activities, schedules, productivity and supervision of the Nursing staff and their assigned team
- Review clinical documentation for completeness, accuracy and consistency for Nursing staff
- Collect QA information from assigned Nurses
- Disseminate policy, protocol change, and ensure implementation across the teams
- Lead regular team meetings and act as the point-person for the Nursing Staffs
- Participate in meetings with the State Department of Health
- Collaborate with project management to ensure departmental performance and operational goals are met-Supervision of the C-YES Regional Nurse Managers (RNMs)-Hiring, training, and developing of RNMs-Complete performance reviews for direct reports and when required indirect reports-Monitor the activities, schedules, productivity and supervision of the RNMs and their assigned team-Review clinical documentation for completeness, accuracy and consistency for C-YES Nursing staff-Collect and review QA information from assigned RNMs-Interpret and disseminate policy, protocol change, and ensure implementation across the teams-Lead regular team meetings and act as the point-person for their assigned cohort of RNMs and the Nursing department as directed by the Project Director-Participate in meetings with the State Department of Health-Support all internal and external audit meetings/schedules-Collaborate with project management to ensure departmental performance and operational goals are met-Conduct scheduled weekly supervision with the RNMs-Participate in the RNM team meetings to ensure compliance with C-YES protocol-Monitor reports generated by C-YES program to support quality implementation and efficient processing of cases-Promote project image and goals through public speaking engagements and local professional presentations-Coordinate regional NE/RNM In-person meetings when required-Attend weekly meetings with Family Support Center leadership and QA/Education leadership-Audit, review and approve expense reports and timesheets-Meet all standards established for this position as outlined in the corresponding annual performance criteria and bonus template for this position-Perform other duties as may be assigned by the Project DirectorMinimum RequirementsMinimum Requirements:
- Current Registered Nurse (RN) license valid in the state of practice is required
- Bachelor's Degree from accredited college or university
- Minimum of 7-10 years of experience working in community settings involving health and human services
- Minimum of 3 years of management experience working in a health-related setting
- 4+ years RN experience in behavioral health care
- Experience working with constituent groups; home health care experience preferred-Bachelor's Degree in Nursing (BSN)
-Minimum of three (3) years of management experience working in a health-related setting
-Minimum of four (4) years of RN experience in a related field (i.e. home care, case management, etc.)
-Minimum of seven (7) years of experience working in community settings involving health and human services
-Active and valid RN licensure in the state of New York
-Valid NYS identification
-Prior experience working with constituent groups.
-Leadership skills including the ability to support and motivate a team of diverse staff.
-Ability to perform comfortably in a fast-paced, deadline-oriented work environment.
-Ability to work effectively with technology in the workplace.
-Ability to adapt to new technology and process changes and support others with such changes.
-Knowledge of the theory, principles, and practices of general professional nursing
-Knowledge of state and agency laws and regulations governing general professional nursing practices
-Excellent organizational, interpersonal, written, and verbal communication skills
-Ability to communicate and work effectively with diverse populations.
