Great Organizational Culture!This position is Monday-Friday, 9:00 am- 5:30 pm with possible WFH flexibility.Job Purpose: To assist in the development and continual improvement of the Access to Healthcare Network Medical Discount Plan Department. The main role of the care coordinator is to case manage AHN members. This entails ensuring that our members are meeting their obligations to AHN and to our providers. A care coordinator will also advocate and problem solve on behalf of our members to ensure that they receive a continuum of care and support. Ensuring this continuum of support requires that a care coordinator help our members navigate the complex health care system and guide them through the treatment plan laid out by their health care provider(s).Key Responsibilities and Accountabilities:
  • Ability to be both compassionate and professional while serving our population.
  • Create and maintain positive relationships with AHN clients based upon a foundation of mutual trust and respec
  • Create and maintain positive working relationships with AHN program partners and providers based upon a foundation of diligence, timeliness and follow
  • Return all phone calls, emails or written correspondence with 48 hours of receipt
  • Process referrals for client with 48 hours of receipt unless otherwise noted by a provider and in accordance with AHN policies and procedures.
  • Gather information needed to price surgeries and have the ability to explain to member's costs and potential scenarios that will alter costs.
  • Gain clear understanding of CPT and ICD codes, medical billing, be comfortable contacting clients in regards to balances and money owed to providers.
  • Gain a mastery of Access to Healthcare Network services available to our members.
  • Be able to use the AHN Salesforce database to locate information.
  • Process, maintain and gather protected health information from clients in a manner compliant with all HIPPA regulations.
  • Ensure that the CC's member's Salesforce database entries are updated regularly.
  • Maintain proper notes and documentation as they become available.
  • Assist members to navigate the health care system and process, while ensuring that no direct medical advice is provided.
  • Be able to multitask and critically think though a variety of situations.
  • Be able to use the phone system to answer calls, transfer calls and answer voicemails.
Other Requirements:
  • Must hold CHW Certification or successfully complete CHW course within 90 days
  • Perform general administrative duties for Access to Healthcare Network when needed and/or requested.
  • Field inquiry calls from interested parties, including potential members and employers. Explain program requirements, make timely disposition of calls and take appropriate actions on inquiry calls and referrals.
  • Effectively and confidently provide program specific orientation for new AHN members as needed
  • Gain clear and working knowledge of social services available throughout our community, both governmental and private.
  • Abide by HIPAA requirements
  • Gain a clear and basic knowledge of Access to Healthcare Network Services
  • Gain a basic and clear working knowledge of the Mitel phone system
  • Gain a basic and clear working knowledge of Salesforce
  • Ability to be both compassionate and professional while serving our population
  • Ability to be both compassionate and professional with staff
  • Must attend staff meetings and be willing to meet with manager on a consistent and regular basis
  • Ability to adapt to changing work environment and duties
  • Other duties as assigned
Education Minimum:
  • High School Diploma; AA Degree Preferred
Experience Minimum:
  • Three years of experience working in health care related field
  • Two years' experience working with databases and software programs
  • Excellent customer service skills
  • Excellent communication skills, both oral and written
  • Excellent organizational skills
Experience Preferred:
  • Two years' experience working with socioeconomically disadvantaged populations
  • Bilingual English/Spanish mandatory
  • Extensive experience working with data bases and software programs such as Microsoft Word, Excel and Power Point
  • Excellent customer service skills
  • Be able to stand between 3 - 6 hours per day
  • Be able to sit at a computer for up to 4 - 6 hours per day
  • Be able to withstand hand/wrist deviation and repetition
  • Must have a valid driver's license and be able to drive the AHN Company Car when needed
  • Must be able to lift, carry up to 30 pounds with a fair consistency
  • Flexibility; Each department selects the hours of work schedule for the department staff based on the department's needs. This schedule may change at any time according to the department's needs.
  • Required to pass a background check through the State of Nevada
