Patient Health Navigator
Position DescriptionOverview
The Patient Health Navigator reports to the Director of Care Management, with accountability for providing strategy, judgment, organization, and evidenced-based analysis to influence decisions, and directly to meet CINQCARE requirements. At CINQCARE, we don’t have patients or customers – we have Family Members.Primary Responsibilities
The Patient Health Navigator (PHN) will have the following responsibilities:
  • Care Management: A Patient Health Navigator is responsible for coordinating and managing the care of individuals with complex healthcare needs. This includes comprehensively assessing the individual's physical, behavioral, and social needs, and developing a care plan in collaboration with the individual and their healthcare team.
  • Service Coordination: Patient Health Navigators facilitate access to services and support that are necessary to meet the individual's health and wellness needs. They assist individuals in navigating the healthcare system, connecting them with appropriate healthcare providers, and coordinating appointments and referrals.
  • Advocacy: Patient Health Navigators act as advocates for individuals, ensuring that their preferences and needs are considered when making healthcare decisions. They also help individuals and families understand their rights and responsibilities within the healthcare system.
  • Communication and Collaboration: Patient Health Navigators facilitate effective communication and collaboration between the individual, their healthcare providers, and other stakeholders involved in their care. This involves sharing pertinent information, coordinating care transitions, and promoting the integration of medical, behavioral, and social services.
  • Education and Support: Patient Health Navigators provides education and support to individuals and their families, helping them to understand their healthcare conditions, treatment options, and self-management strategies. They may also provide resources and referrals to additional support services in the community.
  • Monitoring and Evaluation: Patient Health Navigators continuously monitor the individual's progress and outcomes, tracking their healthcare utilization, adherence to treatment plans, and overall well-being. They evaluate the effectiveness of the care plan and make necessary adjustments to ensure the individual receives appropriate and timely care.
Qualifications
The Patient Health Navigator should have the following qualification:
  • Education:
  • Bachelor’s degree in a relevant field such as healthcare administration, social work, nursing, or a related discipline.
  • A bachelor’s level education or higher in any field with one year of experience working directly with persons with behavioral health diagnoses or
  • A credentialed Alcoholism and Substance Abuse Counselor (CASAC)
  • Associate degree in human services, with at least 5 years of qualifying experience, required.
  • Experience:
  • Previous experience in care coordination or case management required.
  • Working background in behavioral health and substance abuse illness preferred.
  • Familiarity with primary care practice preferred.
  • Proficiency in all Microsoft Office.
  • Knowledge: A good understanding of healthcare systems, services, and resources is essential. Knowledge of healthcare regulations and policies, as well as relevant laws and regulations, is also beneficial.
  • Communication and Interpersonal Skills: Patient Health Navigators need strong communication and interpersonal skills to effectively interact with individuals, families, healthcare providers, and other stakeholders. They should be able to listen actively, communicate clearly, and collaborate with multidisciplinary teams.
  • Critical Thinking and Problem-Solving: Patient Health Navigators must have the ability to assess complex situations, identify needs, and develop appropriate care plans. Strong critical thinking and problem-solving skills are essential to address individual's healthcare challenges and coordinate resources effectively.
  • Organizational and Time Management Skills: Patient Health Navigators often manage multiple cases and tasks simultaneously. Strong organizational skills and the ability to prioritize, manage time effectively, and meet deadlines are crucial to succeed in this role.
  • Cultural Sensitivity: Patient Health Navigators must have a cultural sensitivity and be able to work with diverse populations. This includes understanding and respecting individuals' cultural, ethnic, and socioeconomic backgrounds to provide appropriate care and support.
