Welcome to a medical center where you're the center of attention.Pay range: $95,971.00 - $143,936.00Under the general direction of the Manager of Orthopedic Programs, the Patient Navigator is responsible and accountable for planning, organizing, and assisting the operations of the orthopedic program to achieve and improve patient outcomes, provisions of care and services. The Patient Navigator will be the primary person to prepare patients preoperatively, support them after surgery and seek to optimize patient care throughout the continuum through education, referral management, and utilization of resources. The Patient Navigator communicates and collaborates effectively with the interdisciplinary team to coordinate patient care, ensure continuity of care, and foster excellent patient experiences.
  • Provide or foster medical and social optimization of orthopedic patients before their scheduled elective surgery within scope of practice.
  • Gather information on patient history, labs/test results, specialist consultations, address patient escalations and clearly document assessments and plans of care.
  • Complete patient assessments to help identify post-acute care and readmission risk
  • Coordinate care and discharge planning with the surgeon and other members of the interdisciplinary team.
  • Ensure adherence to orthopedic protocols, clinical guidelines, monitor and trend patient outcomes.
  • Update and maintain orthopedic materials for both staff and patients. Provide ongoing staff education.
  • Provide patient education and assist patients in understanding their diagnosis, intervention, and available resources.
Qualifications:
  • Graduate of an accredited school of registered nursing, BSN required OR graduate of an accredited school of physical therapy, DPT required. For nurses, Orthopedic Nurses Certification (ONC) strongly preferred or willingness to obtain within 3 years of hire.
  • Requires current Washington State Registered Nurse License or Physical Therapy License. Healthcare Provider CPR certification is also required.
  • Minimum of 3 years orthopedic experience required within a clinic, ambulatory surgery center, or hospital. Experience in surgical pre-optimization and/or case management preferred.
  • Familiar with electronic medical record system, Epic preferred. Must have working-level knowledge of the English language, including reading, writing and speaking English. Basic experience with Microsoft Office Suite including Word, PowerPoint and Excel required. Experience leading instructional classes and presentations recommended.
  • Nursing experience preferred.
