We are an organization designed to not only dedicate ourselves to the care of our residents, but also to our valued employees.Columbine Commons Health and Rehab/Assisted Living is located at 1325 Main Street Windsor, Colorado and offers furnished private rooms designed to provide comfort for individuals requiring short-term rehabilitation following hospitalization, long-term care or end of life care/hospice care. Our unique campus provides 2 skilled nursing facilities as well as an assisted living facility.We are accepting applications for a Skilled Nursing Administrator/Executive Director who would be responsible for the following:
  • Plans, directs, coordinates, and evaluates the overall operations of the facility in keeping with the highest standard of care by performing the following duties personally or through subordinate supervisors.
  • Administers and supervises all aspects of the operation of the Facility in accordance with policies set by the governing board and implements those policies, objectives, and philosophies regarding patient/resident care by all departments.
  • Develops and implements preparation of all required records for the operation of the Facility in a professional and business-like manner and analyzes those records to assure economic operation of the Facility.
  • Facilitates compliance with all applicable State and Federal Regulations governing Skilled Nursing Facilities.
  • Oversees and assists department managers/supervisors with the development and implementation of procedures and policies related to staffing and staff development in each department.
  • Coordinates activities of the Facility staff to prevent overlapping of services and duplication of functions, responsibilities and/or supervision.
  • Develops a cooperative relationship with other health care facilities for exchange of information and services. Works with the community agencies to develop an understanding of the programs of the Facility.
  • Reports to the governing board at least quarterly on the Facility operations and its financial condition.
  • Determines that care of patients/residents is maintained at the highest professional level and meets all standards prescribed by regulatory agencies and the governing board.
  • Develops and implements, with the assistance of all department heads, standards for employee conduct while on duty, working hours and rates of pay.
  • Promotes patient/resident's right to fair treatment, individuality, privacy, property, civil rights and the right to lodge a complaint.
  • Oversees department managers/supervisors to ensure annual in-services are conducted with regards to resident's rights.
  • Coordinates functions, responsibilities, and/or supervision of staff.
  • Ensures efficient systems for communication to departments as applicable regarding admits, discharges, room holds, the level of care, room/unit changes.
  • Coordinates Quality Improvement Committee meetings and Partner with Department Managers for the Quality Assurance of each department for ongoing assessment of medical and psychosocial needs of residents.
  • Ensures processes are in place to conduct resident care conferences and that these are appropriately reviewed.
  • Oversees billings, collection services, general accounting procedures, and posting of all charges.
  • Evaluates and implements annual facility/departmental budgets.
  • Oversees policy for food and operational supply purchases.
  • Participates with Management Office for selection of vendors and contracts for services.
  • Enforces the Facility is "Resident Centered".
  • Abide by all HIPAA regulations.
Administrator Requirements
  • Bachelor's degree from a four-year college or university; or five years or more of work experience in sub-acute or rehab; or equivalent combination of education and experience.
  • Must fulfill Administrator license requirements in the State of Colorado and hold a current license.
  • Must have a working knowledge in the following areas:
  • Administrative principles and functions
  • Gerontology
  • Psycho-social sciences
  • Financial management
  • Legal aspects of care
  • Standards and Regulations of practice
Compensation and Benefits
  • Starting wage between $110K - $150K (depending on experience)
